My deliverables were very limited because of the strict level of privacy that I was required to maintain while working with the students. The only real notes I can include are some basic physical therapy rehabilitation exercises I was asked to run through with some patients. All of the notes I took are on the students actual physical therapy file and so I cannot share those. However, I have included a blank exercise sheet and explain how I went about taking notes with the students I worked with.

The first three images are example of rehab exercises that Polly Jones, the physical therapy aid, would give to me for specific students I was assigned to that day. All the exercises are really simple and there aren’t a lot of them. When you are working with preschoolers you are limited in the amount of time you can work with them for a couple reasons. First, you can’t take them out of class for too long (maximum of 15 minutes) and second, because the kids have a really short attention span. Over the course of my project I was able to work with over a dozen different children.

The fourth image is of an actual rehab sheet the physical therapist keeps. After gaining confidence of both Polly and Tori Forsyth, the physical therapist, I was given full rehab sheets to fill out and take notes on for the children I would work with. As you can see on the sheet there are different symbols used to short hand the students’ progress. Any additional notes I took I wrote on the back of the page. Most of the time I just would discuss specific things with Polly before leaving the facility so should could add any other important notes. Again I cannot include the actual notes I kept because of legal issues but this form should give you a good idea of how I kept notes on what I was doing.

 I included to photos of the rehab facility at South Preschool so you can get an idea of where we were working with the kids.

​

Lastly, there is an image of a document signed by the school's Physical Therapy Aid, Polly Jones, confirming that I completed the required hours for the project. She also mentions that I helped taking notes on the students progression in their charts.
